Svi koji žele da se vrate sportu posle preležane korone moraju da budu sigurni da bolest nije ostavila posledice na srce i pluća.

Veliki broj sportista bio je zaražen koronom, kao i ljudi koji se sportom bave rekreativno. Svi oni imaju isto pitanje – kada mogu da se vrate treninzima. Odgovor nije nimalo jednostavan i zavisi od kliničke slike bolesti koju su imali.

Stručnjaci iz vodećih asocijacija medicine sporta i sportskih nauka sastavili su preporuke za kliničke preglede koje sportisti nakon preležanog COVID-19 moraju da urade kako bi bili sigurni da ova bolest nije ostavila posledice na srce i pluća, a sažetak ovih preporuka prenosi Udruženje za medicinu sporta Srbije zajedno sa Udruženjem za sportsku kardiologiju Srbije.

Ove preporuke važe i za rekreativce i za profesionalne sportiste.

Povratak sportskim aktivnostima mora da se odredi individualno za svakog sportistu u odnosu na klinički tok COVID-19. Iako su sadašnje preporuke usredsređene pre svega na procenu srca i pluća, s obzirom na kompleksnost COVID-19 bolesti, nekada je potrebna sveobuhvatnija evaluacija koja uključuje psihološki status i ispitivanje mišićne funkcije.

Za sportiste koji su asimptomatski i negativni na COVID-19, povratak treningu je dozvoljen bez dodatnih ispitivanja, jer nema jasnih dokaza o postojanju bolesti, samo sumnja. Međutim, asimptomatski sportisti koji imaju pozitivan test na COVID-19 (aktivna infekcija) treba da se uzdržavaju od vežbanja u trajanju od najmanje 2 nedelje od datuma pozitivnog testa i slede stroge smernice izolacije.

Ako sportisti ostanu asimptomatski, postpeno vraćanje treningu treba da bude pod nadzorom timskog lekara ili doktora koji se bave
sportskom medicinom. Za asimptomatske sportiste sa otkrivenim COVID-19 antitelima kao odgovor na prethodnu infekciju, preporučuje se slična procena kao kod simptomatskih sportista sa pozitivnim testom na COVID-19 i treba sprovesti kardiovaskularno ispitivanje.

Za sportiste koji su pozitivni na COVID-19 i imali su blage ili umerene simptome, preporučuje se minimum 2 nedelje prestanka bilo kog treninga/vežbanja od trenutka pojave simptoma. Ne zna se tačno da li je povećan rizik od oštećenja miokarda u vidu miokarditisa, kod onih sportista koji su bili hospitalizovani u odnosu na one koji su bili u kućnoj izolaciji, ali obe grupe treba da urade kardiovaskularni pregled u kombinaciji sa srčanim biomarkerima i dijagnostičkim procedurama – ulrazvuk srca. Dalja dodatna ispitivanja sa srčanom magnetnom rezonancom, ili testom opterećenja zavise od slučaja do slučaja.

Za one koji su bili hospitalizovani sa COVID-19, ali čiji su srčani biomarkeri i dijagnostičke procedure npr. ultrazvuk srca bile bez pozitivnog nalaza, preporučuje se najmanje 2 nedelje odmora nakon odsustva simptoma pre nego što se podvrgnu pažljivoj
kliničkoj kardiovaskularnoj proceni praćenim postepenim nastavkom vežbanja.

Očekuje se da sportisti postepeno povećavaju intenzitet i trajanje treninga nakon preležane bolesti. Tokom ovog postepenog povratka sportu, treba svakodnevno pratiti srčanu frekvenciju, intenzitet opterećenja, spavanje, stres, umor i bol u mišićima, zajedno sa psihološkom spremnošću za povratak sportu. Vraćanje vežbanju i treningu mora da ide postepeno kroz nekoliko koraka i ne bi trebalo da počne pre 7 dana od nestanka simptoma, poželjno i duže.

Prvo krenuti 2 dana sa 15 minuta aktivnostima niskog intenziteta (hodanje, brzo hodanje, plivanje, vožnja bicikle, bez treninga opterećenja) održavajući srčanu frekvencu na oko 70% HRmax. Naredni dan početi sa 30 minuta treninga povećanjem opterećenja nabrojanih aktivnosti na oko 80% HRmax, npr. lagani džoging. Sledećih dana početi sa povećanjem dužine trajanja treninga u rasponu od 45/60 minuta, ali na opterećenju koje ne prelazi 80% HRmax. Sve dok ne prođe barem 15 dana od nestanka simptoma, nije preporučljivo vratiti se treningu gde će se prvi put postići opterećenje od 100 HRmax.

Kako bi se sprečile tragične posledice po povratku na trening posle preležanog kovida, udruženja i sportske federacije sastavile su protokol za procenu sportista pre povratka na trening.

Protokol se zasniva na primeni 12-kanalnog EKG-a i ehokardiografije (EHO), međutim zbog specifičnosti novog virusa postoje manje
modifikacije kada su u pitanju sportisti i baziraju se na kliničkim indikacijama.

1. Sportisti koji nisu imali simptome COVD-19 infekcije mogu se vratiti treninzima bez dodatnih analiza.

2. Sportisti koji su imali blage do umerene simptome COVID-19 infekcije (koji su se lečili i oporavili kod kuće), treba da prođu RTP. Neki od njih su imali pozitivan PCR test , a neki samo osnovanu sumnju na COVID 19 infekciju. Za one koji su se potpuno oporavili i nemaju kardio-vaskularne simptome (10 dana od pojave simptoma, a 7 dana potpunog oporavka), preporučuje se klinička procena. Neophodan je fizikalni pregled i detaljna anamneza. U fokusu je prisustvo simptoma: bol u grudima, dispnea, palpitacije, vrtoglavica u naporu, sinkopa, tahikardija, novi šum na srcu, pukoti na plućima i gušenje. Pored toga minimum koji se predlaže su EKG i EHO pre povratka sportu. Ako postoje abnormalnosti sportista se upućuje na magnetnu rezonancu (MRI) da bi se isključio miokarditis. Nakon toga se upućuje na ergospirometriju i 24h Holter EKG monitoring.

3. Sportisti sa perzistentnim simptomima COVID – 19 infekcije kojima je potrebno više od 14 dana da se oporave treba da prođu detaljna ispitivanja; pregled, 12 kanalni EKG i MRI radi procene za miokarditis. Ako je nalaz MRI uredan upućuju se na ergospirometriju i
24h Holter EKG. Ukoliko ne mogu da urade maksimalni test potrebno je dalje praćenje.

4. Sportisti koji su imali teži oblik COVID -19 infekcije koja je zahtevala hospitalizaciju, treba da prođu kompletan protokol za RTP

Većina sportista sa blagim do umerenim oblikom COVID-19 infekcije potpuno će se oporaviti bez potrebe daljeg ispitivanja respiratornog sistema. Neki sportisti, međutim, imaće i dalje produženi kašalj, dispneu i nakon izlečenja, posebno tokom vežbanja. Smatra se da će kod većine simptoma potpuno nestati tokom 4 nedelje. Ukoliko postoji odstupanje od oporavka i pogoršanje ili pojava novih simptoma (produktivni kašalj, bol u grudima ili gušenje). U tom slučaju je potreban ponovni prekid programa treninga i klinička procena (RTG pluća, pluća, D-dimer i procena disajne funkcije kod pulmologa). Potrebno je isključiti pogoršanje pneumonije, emboliju pluća i bronhoopstruciju. Simptomi mogu da budu vrlo slični srčanim smetnjama tako da je potrebno detaljno ispitivanje uz već pomenuto kardiološko.

Kod sportista lečenih u bolnici zbog COVID-19 infekcije sa respiratornim simptomima, neophodna je radiografska potvrda pneumonije. Takođe podaci ukazuju na veliku učestalost trombotičnih događaja posebno embolije pluća. Sva ova ispitivanja su neophodna jer je nastavak bavljenja sportom mogu samo ako prestane nedostatak vazduha i bol u grudima.

Dakle sportisti koji su lečeni od COVID 19 pneumonije pre RTP treba da prođu:

1. Kontrolni snimak pluća;

2. Proveru disajne funkcije (spirometrija i difuzija);

3. Ergospirometriju za utvrđivanje uzroka smetnji u disanju tokom napora.

Hronični post-COVID respiratorni simptomi mogu biti: dispnea u naporu, nova pojava kašlja i sviranja u grudima ili bol u grudima. Na žalost veliki je broj neotkrivenih slučajeva i čak nove pojave astme. Neophodno je strogo pridržavanje svih mera i propisane terapije, a
u slučaju kolapsa pluća ili bolova u grudima i fizikalnog tretmana.

1. Sportistima bez simptoma i znakova COVID 19 infekcije nije potrebno pulmološko ispitivanje

2. Sportistima sa blagim do umerenim simptomima koji su se potpuno oporavili (7 dana bez simptoma, a najmanje 10 dana od pojave simptoma), nije potrebno pulmološko testiranje.

3. Sportisti sa produženim respiratornim simptomima duže od 14 dana treba da se podvrgnu testovima za isključenje tromboembolijskih komplikacija uključujući: RTG pluća, EKG, biomarkere inflamacije, miokardne nekroze i embolije (C-reaktivni protein, hs-cTnT i D dimer) i plućne funkcije. U slučaju sumnje na emboliju pluća indikovan je CT toraksa sa pneumoangiografijom. Ukoliko nema dokaza ovih oboljenja
indikovana je ergospirmetrija uz merenje saturacije O2.

4. Sportisti koji su lečeni u boonici zbog izraženih respiratornih komplikacija tokom COVID 19 infekcije zahtevaju kompletan protokol za RTP

Kada da se vratimo treninzima posle preležane korone i koje preglede da obavimo?

Svi koji žele da se vrate sportu posle preležane korone moraju da budu sigurni da bolest nije ostavila posledice na srce i pluća.

Veliki broj sportista bio je zaražen koronom, kao i ljudi koji se sportom bave rekreativno. Svi oni imaju isto pitanje – kada mogu da se vrate treninzima. Odgovor nije nimalo jednostavan i zavisi od kliničke slike bolesti koju su imali.

Stručnjaci iz vodećih asocijacija medicine sporta i sportskih nauka sastavili su preporuke za kliničke preglede koje sportisti nakon preležanog COVID-19 moraju da urade kako bi bili sigurni da ova bolest nije ostavila posledice na srce i pluća, a sažetak ovih preporuka prenosi Udruženje za medicinu sporta Srbije zajedno sa Udruženjem za sportsku kardiologiju Srbije.

Ove preporuke važe i za rekreativce i za profesionalne sportiste.

Povratak sportskim aktivnostima mora da se odredi individualno za svakog sportistu u odnosu na klinički tok COVID-19. Iako su sadašnje preporuke usredsređene pre svega na procenu srca i pluća, s obzirom na kompleksnost COVID-19 bolesti, nekada je potrebna sveobuhvatnija evaluacija koja uključuje psihološki status i ispitivanje mišićne funkcije.

Za sportiste koji su asimptomatski i negativni na COVID-19, povratak treningu je dozvoljen bez dodatnih ispitivanja, jer nema jasnih dokaza o postojanju bolesti, samo sumnja. Međutim, asimptomatski sportisti koji imaju pozitivan test na COVID-19 (aktivna infekcija) treba da se uzdržavaju od vežbanja u trajanju od najmanje 2 nedelje od datuma pozitivnog testa i slede stroge smernice izolacije.

Ako sportisti ostanu asimptomatski, postpeno vraćanje treningu treba da bude pod nadzorom timskog lekara ili doktora koji se bave
sportskom medicinom. Za asimptomatske sportiste sa otkrivenim COVID-19 antitelima kao odgovor na prethodnu infekciju, preporučuje se slična procena kao kod simptomatskih sportista sa pozitivnim testom na COVID-19 i treba sprovesti kardiovaskularno ispitivanje.

Za sportiste koji su pozitivni na COVID-19 i imali su blage ili umerene simptome, preporučuje se minimum 2 nedelje prestanka bilo kog treninga/vežbanja od trenutka pojave simptoma. Ne zna se tačno da li je povećan rizik od oštećenja miokarda u vidu miokarditisa, kod onih sportista koji su bili hospitalizovani u odnosu na one koji su bili u kućnoj izolaciji, ali obe grupe treba da urade kardiovaskularni pregled u kombinaciji sa srčanim biomarkerima i dijagnostičkim procedurama – ulrazvuk srca. Dalja dodatna ispitivanja sa srčanom magnetnom rezonancom, ili testom opterećenja zavise od slučaja do slučaja.

Za one koji su bili hospitalizovani sa COVID-19, ali čiji su srčani biomarkeri i dijagnostičke procedure npr. ultrazvuk srca bile bez pozitivnog nalaza, preporučuje se najmanje 2 nedelje odmora nakon odsustva simptoma pre nego što se podvrgnu pažljivoj
kliničkoj kardiovaskularnoj proceni praćenim postepenim nastavkom vežbanja.

Očekuje se da sportisti postepeno povećavaju intenzitet i trajanje treninga nakon preležane bolesti. Tokom ovog postepenog povratka sportu, treba svakodnevno pratiti srčanu frekvenciju, intenzitet opterećenja, spavanje, stres, umor i bol u mišićima, zajedno sa psihološkom spremnošću za povratak sportu. Vraćanje vežbanju i treningu mora da ide postepeno kroz nekoliko koraka i ne bi trebalo da počne pre 7 dana od nestanka simptoma, poželjno i duže.

Prvo krenuti 2 dana sa 15 minuta aktivnostima niskog intenziteta (hodanje, brzo hodanje, plivanje, vožnja bicikle, bez treninga opterećenja) održavajući srčanu frekvencu na oko 70% HRmax. Naredni dan početi sa 30 minuta treninga povećanjem opterećenja nabrojanih aktivnosti na oko 80% HRmax, npr. lagani džoging. Sledećih dana početi sa povećanjem dužine trajanja treninga u rasponu od 45/60 minuta, ali na opterećenju koje ne prelazi 80% HRmax. Sve dok ne prođe barem 15 dana od nestanka simptoma, nije preporučljivo vratiti se treningu gde će se prvi put postići opterećenje od 100 HRmax.

Kako bi se sprečile tragične posledice po povratku na trening posle preležanog kovida, udruženja i sportske federacije sastavile su protokol za procenu sportista pre povratka na trening.

Protokol se zasniva na primeni 12-kanalnog EKG-a i ehokardiografije (EHO), međutim zbog specifičnosti novog virusa postoje manje
modifikacije kada su u pitanju sportisti i baziraju se na kliničkim indikacijama.

1. Sportisti koji nisu imali simptome COVD-19 infekcije mogu se vratiti treninzima bez dodatnih analiza.

2. Sportisti koji su imali blage do umerene simptome COVID-19 infekcije (koji su se lečili i oporavili kod kuće), treba da prođu RTP. Neki od njih su imali pozitivan PCR test , a neki samo osnovanu sumnju na COVID 19 infekciju. Za one koji su se potpuno oporavili i nemaju kardio-vaskularne simptome (10 dana od pojave simptoma, a 7 dana potpunog oporavka), preporučuje se klinička procena. Neophodan je fizikalni pregled i detaljna anamneza. U fokusu je prisustvo simptoma: bol u grudima, dispnea, palpitacije, vrtoglavica u naporu, sinkopa, tahikardija, novi šum na srcu, pukoti na plućima i gušenje. Pored toga minimum koji se predlaže su EKG i EHO pre povratka sportu. Ako postoje abnormalnosti sportista se upućuje na magnetnu rezonancu (MRI) da bi se isključio miokarditis. Nakon toga se upućuje na ergospirometriju i 24h Holter EKG monitoring.

3. Sportisti sa perzistentnim simptomima COVID – 19 infekcije kojima je potrebno više od 14 dana da se oporave treba da prođu detaljna ispitivanja; pregled, 12 kanalni EKG i MRI radi procene za miokarditis. Ako je nalaz MRI uredan upućuju se na ergospirometriju i
24h Holter EKG. Ukoliko ne mogu da urade maksimalni test potrebno je dalje praćenje.

4. Sportisti koji su imali teži oblik COVID -19 infekcije koja je zahtevala hospitalizaciju, treba da prođu kompletan protokol za RTP

Većina sportista sa blagim do umerenim oblikom COVID-19 infekcije potpuno će se oporaviti bez potrebe daljeg ispitivanja respiratornog sistema. Neki sportisti, međutim, imaće i dalje produženi kašalj, dispneu i nakon izlečenja, posebno tokom vežbanja. Smatra se da će kod većine simptoma potpuno nestati tokom 4 nedelje. Ukoliko postoji odstupanje od oporavka i pogoršanje ili pojava novih simptoma (produktivni kašalj, bol u grudima ili gušenje). U tom slučaju je potreban ponovni prekid programa treninga i klinička procena (RTG pluća, pluća, D-dimer i procena disajne funkcije kod pulmologa). Potrebno je isključiti pogoršanje pneumonije, emboliju pluća i bronhoopstruciju. Simptomi mogu da budu vrlo slični srčanim smetnjama tako da je potrebno detaljno ispitivanje uz već pomenuto kardiološko.

Kod sportista lečenih u bolnici zbog COVID-19 infekcije sa respiratornim simptomima, neophodna je radiografska potvrda pneumonije. Takođe podaci ukazuju na veliku učestalost trombotičnih događaja posebno embolije pluća. Sva ova ispitivanja su neophodna jer je nastavak bavljenja sportom mogu samo ako prestane nedostatak vazduha i bol u grudima.

Dakle sportisti koji su lečeni od COVID 19 pneumonije pre RTP treba da prođu:

1. Kontrolni snimak pluća;

2. Proveru disajne funkcije (spirometrija i difuzija);

3. Ergospirometriju za utvrđivanje uzroka smetnji u disanju tokom napora.

Hronični post-COVID respiratorni simptomi mogu biti: dispnea u naporu, nova pojava kašlja i sviranja u grudima ili bol u grudima. Na žalost veliki je broj neotkrivenih slučajeva i čak nove pojave astme. Neophodno je strogo pridržavanje svih mera i propisane terapije, a
u slučaju kolapsa pluća ili bolova u grudima i fizikalnog tretmana.

1. Sportistima bez simptoma i znakova COVID 19 infekcije nije potrebno pulmološko ispitivanje

2. Sportistima sa blagim do umerenim simptomima koji su se potpuno oporavili (7 dana bez simptoma, a najmanje 10 dana od pojave simptoma), nije potrebno pulmološko testiranje.

3. Sportisti sa produženim respiratornim simptomima duže od 14 dana treba da se podvrgnu testovima za isključenje tromboembolijskih komplikacija uključujući: RTG pluća, EKG, biomarkere inflamacije, miokardne nekroze i embolije (C-reaktivni protein, hs-cTnT i D dimer) i plućne funkcije. U slučaju sumnje na emboliju pluća indikovan je CT toraksa sa pneumoangiografijom. Ukoliko nema dokaza ovih oboljenja
indikovana je ergospirmetrija uz merenje saturacije O2.

4. Sportisti koji su lečeni u boonici zbog izraženih respiratornih komplikacija tokom COVID 19 infekcije zahtevaju kompletan protokol za RTP
